Dr. Bruce Bothwell, a dentist in Alameda, California, asked me to construct an informational website to help him attract new patients. The website also needed to provide some interactivity which would allow patients to submit requests for appointments.

Dr. Bothwell had an extensive gallery of images depicting various types of dental conditions and the outcome of successful treatments. These images required scanning and enhancement using PhotoShop. He also provided an array of literature describing the various procedures and treatments he performs, and these were assembled with the images into a handy topical guide that helps patients understand what can be done for problems like theirs.

I used Dreamweaver MX to build ASP pages that handle the contact form page. This page allows patients to enter their contact information and preference for appointment days/times. The application is programmed to generate two email messages. The first email, with the patient's information is sent to the doctor's appointment secretary, and second one is sent to the patient acknowledging that their information has been received.
